eQ Homes COVID 19 - PREVENTATIVE MEASURES

QUESTIONNAIRE TO BE COMPLETED BY ALL TRADES PRIOR TO ENTERING AN EQ HOMES SITE

This webform has been created for all sub-trades that work at any job site managed by eQ Homes. The completion and submission of this form is mandatory daily for attendance at our job sites. This enhanced screening will ensure that we are creating a healthy and safe environment for all team members onsite.

1. Do you currently have any of the following symptoms: fever (over 38° C), coughing and/or difficulty breathing? *
2. Have you been exposed to a person who has a confirmed or probable case of COVID-19 infection? *
If you answered YES to questions 1, or 2, please DO NOT come on site. Please complete Ontario Public Health’s Self Assessment Questionnaire and follow their direction.
3. I acknowledge that eQ Homes requires all site visitors to wear proper PPE on site at all times, this includes wearing a mask and eye protection when I am unable to maintain a 2m separation with others onsite.
